Event Reporting Form - .rtf (173kb)
	Main event form. Used as a summary of chirurgeonal activity for an event. Reports go to Chirurgeon's personal file, Autocrat, Kingdom Chirurgeon, and Marshal(in the event of a fighting injury).
Minor Trauma Form - .rtf (27kb)
	Small information form to be keep by chirurgeons while in the field. Helps record patient information for minor trauma. Each form contains four patient contacts.
Patient Care Form - .rtf (168kb)
	A more in-depth form for use with patients who have more serious injuries or that require some form of outside medical attention(such as an ambulance transport).
Note: All Patient Information is Confidential. These Reports should only be read by the treating Chirurgeon and the Kingdom Chirurgeon (except for the Event Report). All others are strictly prohibited by order of the Kingdom Chirurgeon.
